I had one of those years ago and sold it on Ebay.  It is without a doubt for a '64 Special/Skylark.  '65 uses the standard plate and has a special vinyl covered piece that fits over the radio hole and has two studs with nuts that hold it on from the back. 
'64 Skylark 2dr ht 4 speed, 300-4
'65 GS ht, 4 speed,2-4s,AC,PS,PB,PW,Pseat,Tilt
'66 Skylark 2dr ht 300-2 automatic
'78 Yamaha DT 400 2 stroke
'88 Ford F-150 4x4 (used to be 4x2)
'89 Ford F-350 4x4
'03 Honda Accord Coupe V6 6-speed manual (daily driver)

Do not forget the plate that covers the speaker hole also!I have both radio and speaker will try and post pics.A rarer option is the heater delete, covers the contorols and plates for the fire wall!

I had one like Ted's and I donated it to the Reynolds project.

I figured that it was fitting for a race car along with a set of reverse light delete brackets that I sent.

Anyone ever hear anymore from those guys?  :dontknow:
